About This Notice
ER of Dallas is committed to protecting your health information. Federal law requires us to maintain privacy and explain our practices. This Notice applies to all records created at our facility: 4535 Frankford Rd, Dallas, TX 75287.
We maintain electronic health records and may transmit your protected health information electronically to other healthcare providers, laboratories, insurance companies, and other entities as described in this Notice.
We may update this Notice at any time. The current version is always available at our facility and at erofdallastx.com.
Protected Health Information
Protected Health Information (PHI) is any identifiable information related to your health condition, treatment, or payment for care, including medical history, exam findings, test results, diagnoses, treatment plans, and billing records.
How We Use Your Information
Treatment
We share relevant information with physicians, nurses, and clinical staff treating you. When your condition requires specialized care, we coordinate with hospitals and specialists.
Payment
We use your information to bill for services and work with your health plan to resolve billing questions.
Operations
Your records support quality improvement, staff training, audits, and accreditation.
Disclosures Without Your Permission
Law permits or requires disclosure for:
- Legal compliance — court orders, subpoenas, federal/state/local law
- Public health — disease prevention, injury reporting, adverse reaction tracking
- Abuse or neglect — reports to appropriate government agencies
- Oversight — audits, investigations, inspections
- Law enforcement — victim identification, death reports, lawful requests
- Preventing serious harm — imminent threats to health or safety
- Organ/tissue donation — procurement organizations, eye banks
- Death investigations — medical examiners, coroners
- Workers’ compensation — work-related injury or illness claims
- Military — armed forces personnel as command requires
- Government functions — national security, intelligence, protective services
- Correctional facilities — inmate health and safety
When We Need Your Permission
Disclosures not described above require your written authorization. You may revoke authorization anytime by written request. Psychotherapy notes, paid marketing, and sale of information always require written authorization.
Your Rights
- Access records — Request copies in writing. We respond within 15 business days. Reasonable fees may apply.
- Request corrections — Ask us to amend inaccurate or incomplete information.
- Request restrictions — Ask us to limit disclosures. We must honor requests to withhold information from health plans for services you paid out of pocket.
- Communication preferences — Specify how and where we contact you.
- Disclosure history — Request an accounting of disclosures from the past six years. The first request annually is free.
- Paper copy — Request a printed copy of this Notice anytime.
Breach notification — We will notify you if your unsecured information is breached.
Our Responsibilities
ER of Dallas must protect your health information, follow this Notice, accommodate reasonable requests, and notify you of any breach.
Filing a Complaint
File complaints with us or the U.S. Department of Health and Human Services. No retaliation for filing.
